SYNOD JOURNEY REGISTRATION & FORMS


FORMS DUE!

Health and Release Form: Due July 10

This form is required for each participant: youth AND adults!

Special Needs Form: Due July 1
Each congregation group is asked to fill out this form to share dietary, physical, mental and emotional needs your group may have that would be helpful for Synod Journey leadership to know in their planning. All information is confidential. You are also welcome to contact Catherine directly. If no form is received by July 1, we will assume there are no special needs for your group.

CHANGES TO YOUR REGISTRATION
If you would like to make a change to your registration, use the Changes Form linked at the blue button above. 

Additions
Youth and adults may be added as space allows. Catherine will confirm we have room for the addition when this form is received, and the deposit will be due within two weeks.

Substitutions
You may make same gender substitutions up until the time you arrive at UWS! Please contact Catherine first if it is a different gender substitution to make sure they fit in your housing configuration.

Cancellations
If you have a youth or adult cancel and you do not have a substitute to take their place, please use the Changes Form to submit their name. Please remember that all payments are non-refundable if you do not have a substitution ($150 until June 15, $350 after June 15).

Background Checks for Synod Journey Adult Leaders
All adult leaders  are required to submit to a Background Check to participate in the Synod Journey.  Background checks will take place in March/April, after Synod Journey registration has closed. You will receive an email from Active Screening with the subject line "NE MN Synod Journey Background Screening Invitation". Follow the instructions and link in that email to submit your information for your background check. Forms only take 2-3 minutes to fill out.  ​Background checks performed for other organizations cannot be used in place of this one.

The NE MN Synod uses Active Screening to process the background checks. Please read the NE MN Synod Journey Background Check Guidelines for information on what checks we will do and how we will handle your information.
